Kash Dolls Recovery Suites Policy

*KASH DOLLS POST SURGERY INTAKE FORM*

*Waiver of Liability*

 

This agreement releases Kash Dolls Recovery Suites from all liability relating to injuries that may occur during your stay. By signing this agreement, I agree to hold Kash Dolls Recovery Suite entirely free from any liability, including financial responsibility for injuries. (Regardless of whether injuries are caused by negligence)

 

I also acknowledge the risk involved in cosmetic surgery. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries.

 

KASH DOLLS RECOVERY SUITES is NOT a MEDICAL FACILITY and is a service to facilitate my recovery, monitor my recovery, and follow my doctors discharge instructions. All patients are responsible for the choosing of their recovery home. We encourage patients to do their own research and choose the recovery home that will serve their needs.

 

*All patients will have a shared caregiver. Patients do not have an individual, assigned caregiver. *

 

CANCELLATION POLICY

 

  • Deposits will not be refunded under any circumstances

  • Reservations must be paid in full within 7 days of your check in date. In the event that you need to cancel, or that you are cancelled, you will have 365 days to use your credit.

  • If for any reason you need to reschedule, Kash Dolls Recovery Suites does not guarantee that your new date will be available.

  • In the event that you are cancelled or must be canceled, and previously reserved a private suite, we cannot guarantee based on your new dates that a private suite will be available.

 

CHECK-IN/ OUT

 

  • Check-In is available any time after 11:30am.

  • Once you have checked in to our facility, you will not be allowed to leave the facility, unless transported by Kash Dolls Recovery Suites.

  • Kash Dolls recovery Suites does not disclose the address of the recovery home, for the safety of our dolls.

  • You CANNOT Park any vehicles at our facility.

 

  • IF YOU ARE CHECKING IN THE DAY OF YOUR SURGERY, you are responsible for your own transportation to the clinic. Kash Dolls Recovery Suites will pick you up AFTER your surgery. All belongings must be with you at the surgery center.

Medication

 

  • I agree to only take medications that are prescribed to me.

 

Transportation

 

  • I understand that by allowing Kash Dolls Recovery Suites to transport me, I release Kash Dolls Recovery Suites from any and all liability, loss, damages, claims or actions to the maximum extent permissible by law, arising out of such transportation.

  • Kash Dolls recovery suites will pick up from Mia Aesthetics, the airport, and certain locations that must be previously discussed.

  • If you need transportation to any other locations example- Walmart, HEB etc.…. There will be an additional transportation flat fee of $25

 

Meals

 

  • Kash Doll Recovery Suites will provide breakfast, lunch, and dinner. Patients will be notified when meals are ready. We cannot force a patient to eat. If you choose to skip a meal, Unlimited snacks and beverages are available 24 hours a day.

  • Please advise of any food allergies or special diets, so that we may prepare for your stay.

 

ADDITIONS

 

  • Kash Dolls Recovery Suites assume no liability for lost, misplaced, or damaged valuables or belongings. If you discover that you have left behind something of value, please call us immediately and we will assist you in trying to locate your lost item.

  • Alcohol, drugs/illegal substances or nonprescribed medications are not allowed at the recovery home. Failure to follow this rule, will result in the patient being asked to leave the premises.

  • If in a shared room, please be respectful of others and use headphones when on the phone.

  • Showers are to be taken from 8am-9pm, unless it is the morning of your surgery. The showering process can be quite loud, and this facility is shared, so we would like to be courteous of others.

  • We keep the house pretty cool, please bring a heating blanket.
